Step-by-Step MR Claim Submission Process

1. Uploading Covering Letter

  • Covering letter must be uploaded in the DDO login.
  • After scrutiny, the Trust sends a report to the EHS login.
  • DDOs must immediately forward the scrutiny report for sanction.
  • Use official email IDs for communication.

2. Resubmission of Returned Claims

If a claim is returned due to missing documents or mistakes, correct all objections before resubmitting. Repeated mistakes cause serious delays.

3. Documents for Dependent Family Members

For Service Employees:

  • First page copy of Service Register (attested)
  • Family member declaration (attested by DDO)

For Retired Employees:

  • Attested PPO copy
  • Dependent certificate / Non-drawal certificate

4. Claims for Deceased Employees / Pensioners

  • Family member certificate
  • No Objection undertaking from other family members
  • Essentiality certificate

  • Original bills and “Paid by me” receipts

5. Treatment in Unrecognized Hospitals
