Employee Health Scheme (EHS) Login Requisition – Andhra Pradesh
If you are a Government Employee in Andhra Pradesh who has completed four months of service, you are eligible to apply for your EHS Card. To begin the online application process, creating an Employee Login is mandatory.
Below are the standardized letter templates used to request login creation from the Director of the Employee Health Scheme.
Requisition Templates
Depending on your reporting structure, you may choose one of the following formats:
1. Forwarded Through Headmaster (HM)
- Applicable To: School staff reporting to Headmaster
- Recipient: Director, Employee Health Scheme, Amaravati
- Purpose: Request for creation of Employee Login
- Key Requirement: DDO Signature Mandatory
2. Forwarded Through Mandal Educational Officer (MEO)
- Applicable To: Requests routed via MEO Office
- Recipient: Director, Employee Health Scheme, Amaravati
- Purpose: Request for EHS login credentials
- Key Requirement: DDO Signature Mandatory
Mandatory Employee Details
Ensure the following details are provided correctly:

How to Submit
Enclosures:
- Self-attested copy of Aadhar Card
- Last three (3) months Salary Slips
- Fill in the Blanks: Enter employment dates and personal IDs carefully.
- Obtain Signatures: Get your signature and forward the letter for DDO approval.
- Dispatch: Send the completed form to the Director of EHS, Amaravati.
